What do we do?
- Providing
support to Vietnamese GPs and other service providers
-
develop a network and strategies for supporting GPs
-
consult to identify needs;

organise forums/ seminars/ to raise
awareness of services available and clients’ needs;

develop a referral system and
provide a single point of contact to provide information and support in
relation to direct client care
-
identify a core primary health care
team to work with GPs and other service providers.
- Improving
access to health care

Organise focus groups, community
consultation to identify clients’ needs/ issues regarding access to
health services
- Facilitate
passage across the health system to assist GPs to access to
information and referral within the health service
- Promoting
and improving population health outcomes: identify priorities for
prevention and develop strategies to address them through health
promotion strategies and projects
- Promoting
community and consumer participation: support consumers and carers to
participate in networks and have direct input into decision making for
primary care and prevention
- Building
partnerships to enable inter-sectoral action to address immediate and
underlying issues impacting on health of target group
- Evaluate
of the project
Proposed outcomes:
Clients
- Be
better informed of service available at the first point of contact
with PHCN;
- Access
to services beyond the first point of contact will be improved through
increased and more complex referrals
- More
opportunities to address their health needs at the preventative stage
- Timely
referral will provide better outcomes for clients
- Through
networking, case conferencing and care plan clients will receive a
comprehensive health care
- Be
able to participate in making decision for their health care
Health professionals
- Be
aware of services available and clients’ needs
- Increase
the use of case conferencing and care plan; social network, and access
to information
- Increase the number and
range of referrals from and to GPs within the health and other
services
