FAIRFIELD PRIMARY HEALTH NURSING SERVICE

Fairfield Primary Health Nursing Service provides comprehensive, skilled community-based nursing services to people living and working in Fairfield local government area and to local primary schools.

Services are provided by Registered and Enrolled Nurses and we work closely with General Practitioners, local hospitals and community-based services.

All clients referred to the service receive a comprehensive assessment of their needs. Services provided include:

·         Wound management

·         Chronic disease self-management

·         Support to people with respiratory illness

·         Support and education to people with diabetes

·         Administration of medication and support with medication management

·         Continence management including catheterisation

·         Community Acute Care including administration of Intravenous Antibiotics

·         Palliative Care, working in conjunction with the Braeside Hospital palliative care team

·         Nursing care following discharge from hospital

·         Hearing clinics for children and eligible adults

·         School screening and health promotion

·         Immunisation

Access to these services is gained through our efficient intake system. Contact 8717 4009 during business hours. Our intake officers will record the information required (client name, address, date of birth, contact phone number, next of kin contact details, brief health history, service requested) and a nurse will be allocated. Anyone can refer to the service, however if medication administration is required a written medical order will be needed.
