Provided
to both patients and other health professionals. These services
include:

Comprehensive respiratory assessment
(monitoring of vital signs, auscultation of lung fields, SaO2, peak flow
and spirometry monitoring)
·
Education of patients/ carers in disease
process, and proper use of medications.
·
Empower patients and careers to self
manage their respiratory disease
·
Provide self management action plans
·
Assist
in the management of acute exacerbations in the home environment
·
Provide smoking cessation counselling for
respiratory patients
·
Liaise with existing respiratory services
such as Pulmonary Rehabilitation
·
Participate as a service provider on
Enhanced Primary Care Plans for respiratory patients
·
Educate and assist General Practitioners
with the development of Enhanced primary Care Plans, Asthma 3+ visit plan
for respiratory patients

Criteria
Patients should met the following criteria
1.
Aged > 15 years with asthma or
diagnosed with Chronic Obstructive Pulmonary Disease
2.
Reside in the Fairfield Local government
area
